Purpose

Undergraduate chapters may be required by their college or university to provide a Certificate of Insurance (COI) in order to remain in compliance with campus rules and organizational registration requirements. This COI is not related to events or activities, but rather serves as proof of general liability coverage for the chapter’s ongoing presence and recognition on campus.

What Is Covered

This COI provides proof of the Sorority’s Commercial General Liability policy and is used solely for administrative compliance purposes—not for specific events, meetings, or fundraisers.

How to Request a COI

To request a COI for university compliance, please complete the form linked below:

Certificate of Insurance Request Form

  • The COI will be emailed to the address provided in the request form

Important Notes

  • This request is not intended for event coverage. If you need a COI for a chapter-hosted event, please use the separate Event COI Request process

  • Be sure to include any specific language or certificate holder information required by the university

  • If the university requires the COI to be sent directly to them, please include their contact email in the form
