What is a W-9 Form:
W-9 Form is used to provide your correct Taxpayer Identification Number (TIN) to the person who is required to file an information return with the IRS to report, for example: Income/Donation paid to you.
How should a Chapter complete a W-9 Form:
1. The form should be completed from the local chapter perspective.
2. The name on Line 1 should be “CHAPTER NAME, Chapter of Sigma Gamma Rho Sorority Inc.”
3. The entity type in Line 3 should be “Other” and enter “Chapter of 501c7 organization” on the line provided.
4. The exempt payee code in Line 4 should be “1”
5. Lines 5 & 6 enter local chapter address
6. Enter chapter’s EIN in the section for “Taxpayer Identification Number.”
7. Chapter Basileus or Financial Officer should sign and date the form
