The purpose of this article is the provide the appropriate steps for reporting the passing of a Soror.


A Resolution of Thanks document can be requested at the time the Soror's passing is reported to ICHQ. 


Reporting the death of a Soror can be done in two ways. The requestor can report the passing via the National website, or a current member can call or submit a Helpdesk ticket to ICHQ via the Member portal.


Keep in mind, if a family member of the deceased is reporting the Omega Rho, a ICHQ staff member can go to the Omega Rho page in iMIS and complete the Omega Rho Form for them.



OPTION 1: 


Step 1. Current members should sign into the National Website

Step 2. Click the SORORS ONLY tab.

Step 3. Select Chapter Resources

Step 4. Click on Chapter Reporting Forms

Step 5. Scroll down and click the Omega Rho Form link.

Step 6: Click the PLUS (+) sign at the top right side to add the information.

Step 7: Select "Yes" in response to Do You Want a Resolution? question.

Step 8: Click SAVE & CLOSE to complete the submission.










OPTION 2 (Accessible only by ICHQ Staff): 


Step 1: Go to ICHQ Membership

Step 2: Click Omega Rho

Step 3. Click the Request submission form hyperlink to open the reporting form. 

Step 4: Click the PLUS (+) sign at the top right side to add the information.

Step 5: Select "Yes" in response to Do You Want a Resolution? question.

Step 6: Click SAVE & CLOSE to complete the submission.
